At Extendicare Cobourg we constantly strive to meet the care needs of our residents, families and our community.

Our professional staff, which includes occupational therapists, physiotherapists, recreation therapists, registered nurses, licensed practical nurses, dietitian, pharmacist and social worker, provide care for individuals with many complex needs. Residents can feel comfortable in knowing that our competent team will efficiently and effectively manage their health and social needs.

Nursing Care 
We achieve our nursing care goals through the Wellness Model of Care. Care goals are determined with the resident and family, as well as other members of the interdisciplinary team. Desired levels of independence, dignity, and personal preference are all part of the planning process, making each resident feel welcome and at home, while meeting their specific needs. This is achieved through Extendicare programs like Health Trac, Guardian Angel and C.A.R.E.

Professional nursing staff are onsite 24-hours a day. Care aides provide hands-on care at the bedside, assisting residents with the essential activities of daily living. Accurate record keeping, as a component of our quality improvement activities, ensures that provincial, regional, and Extendicare standards are met, reviewed and evaluated on a regular basis.

Individuals with Alzheimer’s disease and related dementias can live life to its fullest in a safe, supportive and caring environment.

Dietary Services
We pride ourselves in providing nutritious, home-cooked meals. Changes to our menu are made as a result of the feedback that we receive from the Resident Food Committee.

Our registered dietitian assesses the nutritional requirements of our residents. Together with our residents, families and registered team members, our dietitian develops the best-suited nutritional plan of care.
 
The total dining experience is important to us and is assured through ongoing communication and feedback.

Resident Programs
We believe that physical conditioning increases residents' feelings of independence and self-worth. Working with the restorative care team, staff conduct assessments and together with residents and family members to develop a suitable rehabilitation or restorative care program.

In the comfort of our home, residents actively participate in walking exercises, fitness programs and speech language therapy as necessary.

Our residents are also provided with social and therapeutic programs. Certified activity aides are busy ensuring that residents' physical, emotional, intellectual, social and spiritual needs are being met. Extendicare Cobourg values resident choices and lifestyles, which play a major role in program planning.

Activity programs are offered seven days a week and include: music, shopping excursions, movie nights, bingo, discussion groups, cards and games, theme parties, carpet bowling, art classes and much more. Special occasions, holidays and seasonal events also contribute to the diversity of our programs.
