Nursing Care
At Extendicare Tri-Town we achieve our nursing care goals through the Wellness Model of Care. Care goals are determined with the resident and family, as well as other members of the interdisciplinary team. Desired levels of independence, dignity, and personal preference are all part of the planning process, making each resident feel welcome and at home, while meeting their specific needs. This is achieved through Extendicare programs like Health Trac, Guardian Angel and C.A.R.E.

Professional nursing staff are onsite 24-hours a day. Care aides provide hands-on care at the bedside, assisting residents with the essential activities of daily living. Accurate record keeping, as a component of our quality improvement activities, ensures that provincial, regional, and Extendicare standards are met, reviewed and evaluated on a regular basis.

Individuals with Alzheimer’s disease and related dementias can live life to its fullest in a safe, supportive and caring environment.

Dietary Services

We pride ourselves in providing nutritious home-cooked meals. Our in-house registered dietitian assesses the nutritional requirements of our residents and together with them, their families and registered team members, creates a best-suited nutritional plan of care.

Changes to our menu are made as a result of the feedback that we receive from our Resident Council. The total dining experience is important to us and is assured through ongoing communication and feedback.

Resident Programs
At Extendicare Tri-Town we believe that physical conditioning increases residents' feelings of independence and self-worth as they work hand-in-hand with our rehabilitation team.

Certified activity aides also ensure that residents' physical, emotional, intellectual, social and spiritual needs are being met. Extendicare Tri-Town values residents' choices and lifestyles, which play a major role in program planning.

Activity programs are offered seven days a week and include: dances, social dinners, shopping excursions, movie nights, bingo, discussion groups, cards and games, theme parties, bowling, art classes and much more. Special occasions, holidays and seasonal events also contribute to the diversity of our programs.
